Application Checklist Application Checklist

You will need to compile and provide to us the following items in order to expedite your loan approval. Please note that not all of this information may be required.

Property Information
  • If you are purchasing property, please submit a Purchase and Sale Agreement, or an Offer Purchase may be substituted on a temporary basis.
  • Copy of listing sheet.
  • For condominiums, the "condo documents" which are the master deed, unit deed, declaration of trust and bylaws, and the hazard insurance master policy (you can obtain these from the seller).
  • Verification of source of checks used for deposit money.
  • If refinancing, a copy of the deed, tax bill and insurance bill or condo fee bill.
Employment
  • Most recent 2 years’ IRS W-2’s for previous two years.
  • Most recent 4 weeks’ paystubs or salary vouchers for the most recent 30-day period with a year-to-date earnings indicated.
  • Relocation Benefit Summary, where applicable.
  • Addresses and telephone numbers of employers for past 2 years.
Residences
  • Complete address(es) of all residences for past 2 years, including names and addresses of landlords, if applicable.
  • Copies of 12 months’ cancelled rent checks (copies of front & back of each check) or satisfactory landlord reference verifying amount paid and length of tenancy.
Liquid Assets
  (i.e. checking, savings and/or investment accounts)
  • Most recent 3 months’ bank statements and/or passbook; including institution name and address.
  • Most recent 3 months’ statements of additional assets including names and addresses.
  • Verification of source of deposit.
Installment Loans   (i.e. mortgage, auto, student, personal, boat, etc.)
  • List of outstanding loans, monthly payment and balance due. Include name, account number and address.
Revolving Credit Cards   (i.e. VISA, MC, Optima, Discover, etc.)
  • Complete list of all open credit accounts.
  • Include monthly payments, account numbers, outstanding balances and addresses.
  • Most recent statement of active accounts.
Federal Loans
  • Veterans Administration Certificate of Eligibility (original only or call the VAEligibility Clerk in Manchester, NH at 603.666.7503)
  • Copy of photo I.D. or drivers license, FHA and VA only.
Self-employed borrowers (25% or more ownership of a company) please also include the following:
  • Most recent 2 years’ federal tax returns, K-1s, etc...signed and dated.
  • Year-to-date Profit or Loss Statement prepared by your accountant, signed and dated.
